Apparently people have petitioned Microsoft to fix this, but Microsoft engineers usually rejected the proposals as not conforming to submission standards or as being on the list of frequently proposed ideas that have already been rejected. The reason for the rejection appears to be a concern about operating system balance, coupled with a lack of motivation by Microsoft engineers.

Wow. I didn't realise it could be so hard. However I am using a mac laptop with an excellent keyboard.

It looks like this does not depend on the operation system (Ubuntu 11.04 here), but on the keyboard. I assume some keyboards (my one is a Logitech Cordless) simply don't send any signals on some key combinations, or signals that the drivers can't understand.

These keys work here when combined with two shift keys: ACDEGIJLNRSTUVWZ°!§$%&/()=?`>_Ä'Ü
The other ones don't: BFHKMOPQRXY"$;:Ö*

This is for a german keyboard layout. If I switch to english, I get this:

TE UIC RWN JUS VER TE LAY DG

(The only difference relevant here is the switched z/y.)

Edit: on another computer (OpenSUSE, with a Dell keyboard):
