Expansion on Multiplayer Etiquette
http://www.wesnoth.org/wiki/MP_CodeOfConduct seems to be missing a few commonly accepted norms.
1. ask players if they are ready before starting game
2. not change settings after players confirm ready
3. refrain from talking dirt
4. continue playing even when loosing (surrenders excluded)
5. confirm required add-on's are up-to-date
I'm sure there's more...
As well--and especially considering the deteriorating game conditions--this should be listed boldly on server MOTD.

I think it would be a good thing if there were ready buttons implemented. Most other games have them, basically all joining players have a toggle and the game can only be started once all players clicked it. Until there is a system like that I believe it's best to leave it to players if they want to do this or not. More on that later.1. ask players if they are ready before starting game

The current code of conduct already states: "Abide by common chat rules", "Be polite, especially with people you don't know" and "Don't use profanity in nicks, map titles, labels, chat, etc.".3. refrain from talking dirt

It's in the interest of all players to do this, but it's best to have an ingame feature doing this.5. confirm required add-on's are up-to-date
Bottomline I believe is that you can have a huge list of rules, regulations, codes of conduct, and recommendations, but the huger you make it, the fewer people will actually take the time and read it. Especially since for the majority of the players the code of conduct is typing one very long url in a browser away. Things like asking if players are ready will be giving through via other players and "don't talk dirty" is common sense. For other things a technical solution is by far the best way to go.
